1. Read the declaration that appears below the text. You must read and agree to the declaration in order to submit this application. Use the scroll bar on the right hand side to view the full declaration.
2. Select the tick box next to “I have read and accept the above legal declaration” to accept the declaration. If you do not select this box then your application will not be submitted.
3. Enter any relevant comments you want to provide to the Regulator under “Registration Notes”.
4. Select the “Save” button if you want to check the page for errors, or leave the application and continue it at a later date.
